Written Statements
The FAA, NTSB, Insurance Companies, and other entities routinely request written statements following certain events or to explain a particular medical condition. Additionally, a written statement is required to file an Aviation Safety Report (ASR).

At this time, pursuant to 14 C.F. R. § 67.413(a), you are requested to provide the following information within 60 days for FAA review:
A detailed statement from you regarding your past and present patterns, and future plans of alcohol use and of the circumstances surrounding the offense. If applicable, please include comments regarding abstinence date, current recovery activities (i.e. AA, Birds of a Feather, SMART Recovery, etc.), and quality of recovery.
Many (but not all) requests for a written statement are optional. However, in most instances it is beneficial to provide a statement anytime one is requested or permitted, even if such is not legally required.
All information you provide to the FAA will become part of the respective file and may not be retracted later. Therefore, it is critical that you make all such statements accurate, organized, and thoughtful.
